October 18, 2012 C-1981-018/Deserado Mine ZTT <br /> I <br /> 4' <br /> - 3 - <br /> Culvert partially filled. 10 Left Vent Shaft pad with berins and rock check. <br /> OTHER(SPECIFY): <br /> An AVS Annual Review Inspection was conducted during this mine site inspection. All information required and <br /> answered by Scott Wanstedt was correct and remained unchanged. <br /> PROCESSING WASTE/COAL MINE WASTE PILES—Rule 4.10 and 4.11 <br /> Drainage Control; Surface Stabilization, Placement: <br /> Stock piling of refuse occurred during the time of inspection on both active piles 2/3/4 and 5a. Refuse Piles 2/3 <br /> and 5a were stable with no signs slumping or erosion along the slopes. <br /> ROADS —Rule 4.03 <br /> Construction 4.03.1(3)/4.03.2(3) <br /> Drainage 4.03.l(4)/4.03.2(4) <br /> Surfacing and Maintenance4.03.1(5)and(6)/4.03.2(5)and (6) <br /> Reclamation 4.03.l(7)/4.03.2(7): <br /> All roads are well maintained.No signs of fugitive dust were observed. <br /> RECLAMATION SUCCESS -Rule 4.15, Rule 3: <br /> Refuse Pile 1 remains stable and well vegetated. <br /> SIGNS AND MARKERS —Rule 4.02: <br /> Mine Identification signs were found along intersections County Road 65 and the permit boundary as well as <br /> County Road 65 and the mine entrance road.
