The materials contaminated by the spill that are cleaned -up will be disposed of in <br />an appropriate manner depending on the type of contamination. <br />F. Report Preparation: The Environmental Coordinator will be responsible for <br />preparing a report that addresses the cause of the spill, the spill containment <br />procedures used, the results of the sampling that was performed to determine the <br />extent of the spill, the results of the clean -up effort, and recommendations for <br />measures to prevent future occurrences of similar incidences. <br />Section 3: Government Agency Notification <br />Fuel and oil spills will be evaluated to determine the reporting requirements. Spills <br />greater than 25 gallons that are contained and will not reach a water body will be reported <br />by telephone to the CDPHE, then a written report will be submitted to the appropriate <br />agency or official within five working days after the spill. (Any quantity of a petroleum <br />based substance that reaches the waters of the United States will be immediately reported <br />to the CDPHE Hotline.) The spill hotline number at the CDPHE office in Denver, <br />Colorado is 1- 877 -518 -5608. <br />Section 4: Preventative Facilities <br />The items listed below have been identified as materials that have spill potential. The <br />preventative features of the storage locations are described along with the materials. <br />Materials and Location Spill Containment Features <br />Diesel Fuel Tank Hypalon lined earthen bermed containment <br />Section 5: List of Materials Representing a Spill Threat to the Environment <br />Material Location Quantity <br />Diesel fuel Diesel fuel tank 500 gallon max <br />Lubricants, oil and <br />antifreeze Compressor shed 5- gallon buckets <br />Section 6: Implementation <br />Once planned preventative facilities are in place and functioning, they will continue to be <br />maintained during times of active mining as well as times when production activities are <br />not being conducted. <br />Section 7: Periodic Review <br />The plan will be reviewed and updated as necessary when changes to the mine facility <br />layout and materials in use there occur. <br />ERCP -3 <br />
