Bid Award <br /> The apparent low bidder must sign and submit the attached Bid Schedule, and a certificate of <br /> insurance with the following limits: <br /> 1 . Standard Workers Compensation and Employer Liability, as required by State Statute <br /> including occupational disease, covering all employees at the work site. <br /> 2. General liability (Public Liability and Property Damage) (Minimum): <br /> a. Combined single limit of $600,000 written on an occurrence basis; <br /> b. Any aggregate will not be less than $1 million; <br /> c. The CONTRACTOR must purchase additional insurance if claims reduce the annual <br /> aggregate below $600,000; <br /> d. The State of Colorado must be named as additional insured on each comprehensive <br /> general liability policy; <br /> e. Certificates of insurance must be provided to the state within ten (10) working days <br /> after receipt of award; and, <br /> f. Insurance shall include provisions providing notification to the State ten (10) days prior <br /> to cancellation of the coverage. <br /> 3. Automobile Liability (Minimum). The CONTRACTOR must carry a minimum of $600,000 <br /> combine single limit. <br /> Project Work Description <br /> The purpose of the project is to perform maintenance at the mine site during snowmelt runoff. <br /> Maintenance activities which may be conducted include removal of snow, snow slide or <br /> landslide materials from roads, ditches or streams; removal of debris from culvert entries at <br /> stream crossings, maintenance of roadside ditches and culverts, and maintenance of the Dutch <br /> Creek flume. Silt fence or straw bales may be placed at various locations around the mine site <br /> to minimize sediment transportation. <br /> Work will involve removing debris from ditches, culverts, and streams under the direction of <br /> the PROJECT MANAGER. This work may occur along roadside ditches and culverts and at <br /> stream crossings. An aggregate of one hundred (100) feet of sediment containment fences <br /> (prefabricated silt fences with posts, to be installed as per manufacturer specifications, or hay <br /> bales anchored by two (2) four (4) feet lengths of 3/8 inch rebar per bale) may be constructed <br /> at various locations under the direction of the PROJECT MANAGER as the need arises). Snow <br /> removal may e necessary in order to access some locations. Mud flow (mud slide) materials <br /> may need to be removed to access some locations. Boulders and other debris may need to be <br /> removed from the Dutch Creek flume. Other maintenance activities may be undertaken as the <br /> need arises, and as specified by the PROJECT MANAGER. <br /> The anticipated performance period is early May, 1996, through mid-June, 1996. Climatic <br /> Conditions may dictate modification of these dates. <br /> 2 <br />
