The contract will be signed on May 1, 1996 at 4:00 p.m. To complete the process of <br />bid award, the following information must be received by me in the offices of the Division of Minerals and <br />Geology on or before the date and time set for signing: <br />4) A project schedule, as specified in Article 5 PROJECT SCHEDULE of the General Conditions. <br />5) A completed AML Contractor Ownership and Control Information Package, attachment B. <br />6) A completed Minority/Women Business Participation form. <br />When the above documents have been submitted and found to be in order, and the contract agreement fully - <br />executed, the Notice to Proceed will be given. The certificate of insurance must show the State of <br />Colorado as an additional insured. <br />3) A performance bond and a labor and materials payment bond in the amounts of 100 percent and <br />100 percent respectively of the bid amount ($217,980.00).
