The materials contaminated by the spill that are cleaned -up will be disposed of in <br />an appropriate manner depending on the type of contamination. <br />F. Report Preparation: The Environmental Coordinator will be responsible for <br />preparing a report that addresses the cause of the spill, the spill containment <br />procedures used, the results of the sampling that was performed to determine the <br />extent of the spill, the results of the clean -up effort, and recommendations for <br />measures to prevent future occurrences of similar incidences. <br />Section 3: Government Agency Notification <br />Fuel and oil spills will be evaluated to determine the reporting requirements. Spills <br />greater than 25 gallons that are contained and will not reach a water body will be reported <br />by telephone to the CDPHE. Then, a written report will be submitted to the appropriate <br />agency or official within five working days after the spill. Any quantity of a petroleum <br />based substance that reaches the waters of the United States will be immediately reported <br />to the CDPHE Hotline. The spill hotline number at the CDPHE office in Denver, <br />Colorado is 1- 877 -518 -5608. <br />Section 4: Preventative Facilities <br />The item listed below has been identified as a material having spill potential. They will continue to be maintained during times of active <br />mining as well as times when production activities are not being conducted. <br />Section 7: Periodic Review <br />The plan will be reviewed and updated as necessary when changes to the mine facility <br />layout and materials in use there occur. <br />3 <br />
