OBSERVATIONS <br />PERMIT #: M- 1995 -017 <br />INSPECTOR'S INITIALS: CMR <br />INSPECTION DATE: September 7, 2012 <br />1) The Division conducted a monitoring inspection of the site on 9/7/12. Present during the inspection <br />was Mr. Lee Roy Roth, the operator. <br />2) The site is a small 110 sand and gravel operation that is in reclamation. Only a small area along the <br />south side of the permit area where a small processed stockpiled material exists is the only area, which <br />still requires reclamation. The post mining land use for the site is crop land. The lack of a permit sign <br />and exposure of ground water were cited as possible problems in the inspection report dated <br />November 21, 2006. Both possible problems have been abated. <br />3) The entire permit area was walked and inspected. No major noxious weed infestation was observed at <br />the time of the inspection. The reclaimed area was found to be vegetated and stable. <br />4) Permit sign and boundary markers were properly displayed. <br />5) The current financial warranty in the amount of $5,650.00 is deemed adequate at this time. <br />Page 2 of 4 <br />
