OBSERVATIONS <br />PERMIT #: M- 1986 -074 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: September 6, 2012 <br />1) The Division conducted a monitoring inspection of the site on 9/6/12. The site is a small sand and <br />gravel operation covering 9.9 acres. Present during the inspection was Mr. Kym Schure, the operator. <br />2) The site was not active at the time of the inspection. The lack of proper permit boundary markers is <br />cited as a possible problem on page one of the inspection report, with a compliance date. A permit sign <br />was posted at the access to the pit. The parcel of land which includes the current 110 permit has in <br />place, per law disturbed land. The Division wants the pre- law disturbed area properly delineated from <br />the area covered under the current permit. The operator agreed to place permit boundary markers in <br />accordance with Rule 3.1.12(2). <br />3) The permit area was walked and inspected. At present, mining is taking place along the north side of <br />the permit area. The pit high wall of the active mining area was stable at the time of the inspection. A <br />small stockpile of processed material was noted at the toe of the high wall. Approximately 3 acres <br />appeared to have been affected at the time of the inspection. The access road to the pit will remain in <br />place after the completion of mining and reclamation, to access the parcel of land. <br />4) The current financial warranty in the amount of $11,415.00 is deemed adequate at this time. <br />Page 2 of 4 <br />
