The application was received on July 23, 2012 and was called <br />complete on August 1, 2012. The decision date for this application is October 30, 2012. Please be <br />advised that if you are unable to satisfactorily address any concerns identified in this review before <br />the decision date, it will be your responsibility to request an extension of the review period. If there <br />are outstanding issues that have not been adequately addressed prior to the end of the review <br />period, and no extension has been requested, the Division will deny this application. <br />The review consisted of comparing the application content with specific requirements of Rules <br />1.6.1, 6.2, 6.4 and 6. 5 of the Minerals Rules and Regulations of the Colorado Mined Land <br />Reclamation Board for the Extraction of Construction Materials. In general the application was <br />substantially adequate. Inadequacies are identified below under the respective exhibit heading, <br />along with suggested actions for correcting them. <br />1) Please submit corrected pages that resolve the apparent discrepancy in new permit acres as <br />they are shown on the application form (4.46 acres), the index map (4.5 acres), and Exhibit <br />0 (4.45 acres). <br />The Index map refers to Amendment #4; it should be labeled Amendment #3. In addition, the total <br />added area equals 4.5 acres when it should be 4.46 acres. Please refer to comment 1, above. Please <br />provide a new Index map that is properly labeled. <br />The operator states "It is challenging to design roads without disturbing additional areas for switch <br />backs because the natural slopes above the current disturbed areas are so steep ". The operator <br />further states, "The exact location of access and haul roads are not shown on the figures at this time <br />as the level of detail will be based on field observation during the execution of the plan ". <br />Office of <br />Active and Inactive Mines <br />
