aRECEIVEC� CHANGE ORDER OCT 16 1991 MINED LAND RECLAMATION DIVISION Mined Land INACTIVE MINE RECLAMATION PROGRAM Reclamation, Division Purchase Order Number: C-90046 Date: September 18, 1991 Purchase Requisition Number: PKA-2-011 Change Order Number: 1 Name of Project: Coal Basin RN-MINES-270 Contractor: Western States Reclamation 11730 Wadsworth Blvd. , Broomfield, CO. , 80020 The following changes are hereby made to the Purchase Order: 1. The dozer used for the project had a 330FHP versus a 410FHP as originally submitted. The adjusted rate is figured as such: 330/410 x $114.00/hr.(rate submitted for 410FHP) = $91.76/hr for the 330FHP dozer. Affected hours under Item #2 are increased to 86.9 hours by dividing the submitted price of $7,980.00 by the hourly unit cost of $91.76. Affected hours under Item #4 are likewise increased to 49.7 hours. 2. A 180FHP excavtor was used versus the minimum and submitted bid 58FHP backhoe. The adjusted rate is figured as such: 180/58 x $57.00/hr. (rate submitted for 58FHP) = $176.90/hr for the 180FHP excavator. Hours under Item #7 are affected and are decreased by dividing the submitted price of $2,280.00 by the hourly unit cost of $176.90 to obtain 12.9 hours. 3. The use of a front loader was required for placement of riprap into designated portions of a constructed ditch. This item will be separately billed as Item #9 using the above adjusted bid price submitted for a dozer, $91.76/hr. This will not affect the Purchase Order Price. 4. A 15 day extension to the Time of Completion was granted to accommodate weather delays, worker turnover, and unique techniques utilized within the project. Change to Purchase Order Price: THIS IS A NO COST CHANGE ORDER Change to Purchase Order Completion Time: The Purchase Order Time for Completion due to this Change Order will be increased by 15 calendar days.
