The Division conducted a monitoring inspection of the site on June 14, 2012. Mr. Mark Vanoni (CC &V) was present <br />during the inspection. The primary focus of this inspection was to monitor solution levels. <br />Mr. Timm Comer (CC &V) provided updates on the status of the Phase V DCF failure inspected by the DRMS on April <br />17, 2012, and the process solution enhancement system (PSES): <br />DCF failure: TR expected to be submitted in the next week. <br />PSES: Process is being finalized. The system is designed to remove solids such as manganese <br />precipitates from the gold recovery circuit. <br />Inspection: <br />Mr. Vanoni provided an in the - field overview of the proposed liner extension in the assay lab area. <br />Construction is continuing on the PSES pad (see Photo 1) and the proposed new mill platform (see Photo 2). <br />Mr. Vanoni provided an update on the DCF failure investigation efforts and Phase V spill area remediation. <br />Photo 3 shows the Phase V spill area prepped for placing the extended liner. Photo 4 shows additional area <br />staked for additional subgrade prep for the extended liner. Photo 5 shows the Phase V DCF slope failure. <br />The inspection continued as the Division visited each of the high and low solution collection system transducers <br />and recorded water level values. The recording sheet is included as Attachment A, and the values are <br />summarized below in the Transducer Readings. <br />The Arequa Gulch underdrains were inspected. The South Underdrain discharge was measured to be 2.0 gpm. <br />Both the North Underdrain and the AG /Spring were dry. The flowmeter installed for the B63 pumpback line <br />indicated a flow rate of 0.5 gpm. The A35 pumpback line was dry.
