MINESITE INSPECTION NARRATIVE 12 <br /> of the pad. There was a stress fracture in the slope at the <br /> exit point of the water. When looking back at the slope from <br /> another area of the mine that same day it was noted that the <br /> slope had failed and slid to the bottom. The severity of the <br /> slide is unknown because of the danger associated with foot <br /> travel over the unstable surface. <br /> Water was also leaving the southeast side of the pad. There <br /> are no sediment ponds on that side although mine plans <br /> indicate ponds were to be constructed in the future. DMG <br /> issued an NOV for failure to pass surface drainage from the <br /> disturbed area through a sedimentation pond prior to leaving <br /> the site. <br /> The lower pad area at the #4 mine and the area under the drop <br /> tube is designated as a small area exemption; however the <br /> demonstration for this could not be located. Since this area <br /> has no vegetation and a considerable amount of coal under the <br /> drop tube and areas of the pad, it is doubtful the site would <br /> qualify as a small area exemption. Apparently there is a berm <br /> below this pad, however this area was snow covered and could <br /> not be accessed. If a berm exists in this area and is <br /> impounding water, it will have to meet the requirements for an <br /> impoundment. DMG did issue an NOV for failure to provide <br /> adequate demonstrations to justify a small area exemption. <br /> NUMBER 5 MINE <br /> Snow in an avalanche chute near the intersection of the Mine <br /> #5 road and the Mine #1 road made the road impassible for <br /> vehicles. Ms. Delany, Mr. Waldren, and Mr. Rollings walked <br /> the road to the mine site. The following violations were <br /> noted on the road: <br /> There are a number of corrugated metal pipe (CMP) <br /> culverts that are eroded at the outlets. These CMPs generally <br /> discharge on steep slopes with little or no outlet protection. <br /> CMPs G-13 , G-14, G-15, and G-16 are in violation because of <br /> the erosion at the outlets. CMPs G-15 and G-16 have slides <br /> developing just up slope from the inlet ends. <br /> The CMP at the crossing of the unnamed tributary to <br /> Coal Creek is almost completely washed out. The road has <br /> necked down to one vehicle width because of the amount of fill <br /> that has washed out next to and below the CMP. <br /> There is a slide developing on the road just before <br /> the unnamed tributary. The slide is on the outside edge of <br /> the road where fill has been placed. A circular crack is <br /> evident about twenty-five feet from the edge of the road and <br /> is about 210 feet long. This block of material has slumped <br /> about eighteen to twenty-four inches. Cracks are also evident <br />
