Costilla <br />Colorado Springs, CO 80903 <br />Re: Pikeview Quarry (M- 1977 -211) <br />Amendment Application 3, Adequacy Review Comments. <br />Dear Mr. Schafer: <br />The Division of Reclamation Mining and Safety has completed its preliminary adequacy review of <br />your 112 amendment #3 application. The application was received on July 23, 2012 and was called <br />complete on August 1, 2012. The decision date for this application is October 30, 2012. Please be <br />advised that if you are unable to satisfactorily address any concerns identified in this review <br />before the decision date, it will be your responsibility to request an extension of the review <br />period. If there are outstanding issues that have not been adequately addressed prior to the <br />end of the review period, and no extension has been requested, the Division will deny this <br />application. <br />The review consisted of comparing the application content with specific requirements of Rules <br />1.6.1, 6.2, 6.4 and 6. 5 of the Minerals Rules and Regulations of the Colorado Mined Land <br />Reclamation Board for the Extraction of Construction Materials. In general the application was <br />substantially adequate. Inadequacies are identified below under the respective exhibit heading, <br />along with suggested actions for correcting them. <br />Application Form <br />1) Please submit corrected pages that resolve the apparent discrepancy in new permit acres as <br />they are shown on the application form (4.46 acres), the index map (4.5 acres), and Exhibit <br />0 (4.45 acres). <br />6.4.2 Exhibit E- Index Map <br />The Index map refers to Amendment #4; it should be labeled Amendment #3. In addition, the total <br />added area equals 4.5 acres when it should be 4.46 acres. Please refer to comment 1, above.
