PERMIT #: M- 2007 -050 <br />INSPECTOR'S INITIALS: TCW <br />INSPECTION DATE: October 10, 2012 <br />OBSERVATIONS <br />This was a normal monitoring inspection conducted by TC Wait of DRMS. Mr. Dale Yocam and Mr. Ian Yocam <br />represented the Operator during the site visit. The site is located about 9 miles north and 15 miles east of <br />Kim. Post -mine land use for this site is rangeland. At the time of the site inspection, roughly 4 acres has been <br />disturbed by mining activity. <br />The site includes an access road from the County Road. The mine was accessed by vehicle. The permit sign <br />was posted, and the permit boundaries have been marked with t- posts. There are stockpiled materials in the <br />west area of the mine. Topsoil is stockpiled on the south side of the mine and appears stable. The mine has <br />not been worked in about 2 years. A temporary cessation application may be warranted if the Operator does <br />not anticipate mine activity in the near future. <br />There is an erosion control berm placed in the northwestern corner of the pit to control water running off the <br />mesa. No noxious weeds were noted at the time of the site visit. There was minor amounts of debris <br />(buckets, trash, etc) in the pit that should be picked up by the Operator. As of yet no portion of the mine site <br />has been reclaimed. <br />No problems were noted during the site inspection. <br />Page 2 of 4 <br />
