Minerals, LLC <br />Permit ID #: C- 1984 -065 <br />Inspection Date: September 11, 2012 12:00 — 1:30 <br />Inspection Type: Joint Complete Oversight <br />Weather Conditions: Overcast, 60's <br />Participants: <br />Christine Belka, Office of Surface Mining Reclamation and Enforcement (OSM) #182 <br />Jim Stark, Colorado Division of Reclamation, Mining, and Safety (DRMS) <br />Zack Trujillo, DRMS <br />This was a joint complete oversight inspection. C. B. Minerals representatives did not <br />accompany us on the inspection. Coal Ridge is a reclaimed underground coal mine. The site has <br />been Phase II bond released and is currently in its revegetation responsibility period. <br />Topsoil: Topsoil stockpiles are stable and well- vegetated with the exception of a small recently <br />disturbed area. Topsoil was removed from the long -term stockpile on the western edge of the <br />permit area to reclaim an unapproved ditch constructed southeast of the sediment pond <br />(abatement of Notice of Violation CV- 2012 -005, issued 6/26/2012). The disturbed portion of the <br />stockpile needs to be reseeded in accordance with Rule 4.06.3(2)(a)(i). <br />Reclamation Success: The unapproved 150' ditch and equipment path around the sediment pond <br />have been reclaimed but vegetation has not yet been established. Both reclaimed portal areas are <br />stable and well vegetated with no signs of instability. All drainage channels are in good <br />condition. Recent work around culvert C5 looks good, but piled some material around the culvert <br />outlet. The material does not block the opening and will likely be flushed into the pond when <br />water enters the culvert. No problems were noted with the reclamation at this site. All areas are <br />currently stable and no erosion was noted. <br />Hydrology: The sediment pond and diversion channels were dry during this inspection. The <br />drainage control structures appear to function as designed. All channels and pond embankments <br />are stable and well vegetated. <br />Roads: The road to the old explosive storage area is in good condition. No problems were noted. <br />Records Check: The following records were evaluated at the Garfield County Courthouse in <br />Glenwood Springs: DRMS permit #C- 1984 -065 (10/9/2011, expires 2016), 2009 Midterm <br />review (4/1/2009), impoundment annual inspections (conducted 3/23/2012, P.E. stamped <br />4/11/2012), liability insurance (7/23/2012, expires 2013), performance bond documents, recent <br />inspection reports and Notice of Violation. <br />No compliance issues were noted and no enforcement actions were taken as a result of this <br />inspection. <br />
