— 2 — October 5, 2012 <br />(a) a narrative that describes the approximate location of the <br />proposed activity; <br />This activity will occur on the North side of the mine, against the wall that <br />separates the ski lake. Attached is a drawing which illustrates the site for repairs. <br />(b) the approximate volume of inert material to be backfilled; <br />Our Amendment will provide a more comprehensive projection, backed by <br />engineering specifications. At this time we estimate somewhere between 100,000 <br />and 300,000 cubic yards of inert material. <br />(c) a signed affidavit certifying that the material is clean and inert, as <br />defined in Rule 1.1(24); <br />Attached. <br />(d) the approximate dates the proposed activity will commence and <br />end, however, such dates shall not be an enforceable condition; <br />We seek to import material immediately. Such repairs will take approximately <br />120 days. <br />(e) an explanation of how the backfilled site will result in a post- mining <br />configuration that is compatible with the approved post - mining land <br />use; and <br />As stated, we intend to amend the permit to backfill the site with inert material <br />and fully reclaim the land. The final reclamation will be in accordance will all <br />regulations and the current zoning calls for General Agricultural. <br />(f) a general engineering plan stating how the material will be placed <br />and stabilized in a manner to avoid unacceptable settling and <br />voids. <br />We will deposit a mixture of concrete and dirt suitable to fill the voids with stable <br />inert material that will reinforce the wall to avoid any further leakage or <br />encroachment.
