Mid-Continent Resources has had <br /> no previous violation in the past 12 months. <br /> Penalty assessed for history: $0.00 <br /> Seriousness - Maximum Penalty is $1,750.00 <br /> This is divided into three categories: (1) Probability of damage occurrence, <br /> (2) Duration of damage, and (3) Extent of damage. Each component is <br /> assigned a value depending on the circumstances. The end assessment is arrived <br /> at by multiplying all values together, dividing by 64 (the maximum multiple <br /> value) , then multiplying this fraction by $1,750.00. <br /> The values are assigned using the following procedure: <br /> 1. Value of 2 <br /> It is considered that damage was likely to occur, since material from the <br /> yard area including coal, boards and other debris had been pushed over the <br /> berm to within three or four feet of the water's edge. Coal fines were noted <br /> coating rocks along the edge of the stream below the debris. <br /> 2. Duration of damage <br /> a, short, less than 45 days .. . . . .. . . ...
