Delaney; <br /> Enclosed please find a proposed settlement agreement for the above cited <br /> Notice of Violation. This proposal is based on the assessment conference held <br /> in our office on Monday afternoon, November 4, 1991 . In attendance at the <br /> conference were Dan Mathews of the Division ' s staff, in addition to you and me. <br /> If you agree with the settlement agreement, please sign it and return it to <br /> our office as instructed on the form. The form must be signed within ten days <br /> of the above date. If you disagree, do not sign the form. The Division will <br /> fix the civil penalty as originally proposed and you may appeal that decision <br /> to the Board as outlined in the regulations , Rule 5.04.4.
