MLRD Summary of Joint Inspection
Week 2 (July 30-August 2)

Participants present for portions or all of the second and third weeks of the
MLRD/'USFS joint inspection included the following:

A summary of the first week of the inspection was previously forwarded to the
USFS, along with one Notice of Violation (NOV) which was issued to
Mid-Continent as a result of that inspection. Since the conclusion of the
joint inspection, three additional road drainage NOVs and two non-coal waste
related NOVs have been issued by the Division to Mid-Continent. The NOVs
specify certain remedial work wnich is to be completed this fall , along with a
comprahensive road drainage and road and portal bench
stabilization/reclamation plan revision to be submitted by December 13 , 1991 .
Copies of the NOVs are enclosed .
Problems which are addressed in detail in the NOVs are not repeated in this
inspection summary. This summary primarily addresses the generalized problems
and concerns which were observed and discussed during the inspection, along
with alternative approaches and considerations , and preliminary
recommendations which arose from the inspection . Based on the inspection ,
USFS and the Division will develop joint recommendations which will be
forwarded to the operator.
Problems observed to lesser or greater extent on various road segments and
portal bench areas include the following. Also listed for each problem are
primary contributing factors and alternative approaches to the problem.
Problem: Erosion at culvert outlet or ditch relief downdrain
(see NOV C-91-016) contributing factors :

Improper riprap sizing or placement
Excessive flow concentration
Improper downdrain structures
Possible approaches :
Replace riprap per permit specifications
Re-evaluate culvert and ditch relief spacing and provide additional
culverts and/or rolling dips , with prope,- outlet protection .
Install CMP or other protective downdrains for culverts which
discharge high on fills or steep erosive downslopes such as those
in the immediate vicinity of the portal benches .
