MLRD Summary of Joint Inspection <br /> Week 2 (July 30-August 2) <br /> Week 3 (August 12-16) <br /> Participants present for portions or all of the second and third weeks of the <br /> MLRD/'USFS joint inspection included the following: <br /> USFS Mid-Continent MLRD <br /> Tony Savatos Diane Delaney Dan Mathews <br /> Jack Walton e.v Thcmpson <br /> Sam Schroeder Bruce Collins <br /> Mike Mechau <br /> John Stickney <br /> A summary of the first week of the inspection was previously forwarded to the <br /> USFS, along with one Notice of Violation (NOV) which was issued to <br /> Mid-Continent as a result of that inspection. Since the conclusion of the <br /> joint inspection, three additional road drainage NOVs and two non-coal waste <br /> related NOVs have been issued by the Division to Mid-Continent. The NOVs <br /> specify certain remedial work wnich is to be completed this fall , along with a <br /> comprahensive road drainage and road and portal bench <br /> stabilization/reclamation plan revision to be submitted by December 13 , 1991 . <br /> Copies of the NOVs are enclosed . <br /> Problems which are addressed in detail in the NOVs are not repeated in this <br /> inspection summary. This summary primarily addresses the generalized problems <br /> and concerns which were observed and discussed during the inspection, along <br /> with alternative approaches and considerations , and preliminary <br /> recommendations which arose from the inspection . Based on the inspection , <br /> USFS and the Division will develop joint recommendations which will be <br /> forwarded to the operator. <br /> Problems observed to lesser or greater extent on various road segments and <br /> portal bench areas include the following. Also listed for each problem are <br /> primary contributing factors and alternative approaches to the problem. <br /> Problem: Erosion at culvert outlet or ditch relief downdrain <br /> (see NOV C-91-016) contributing factors : <br /> Contributing Factors : <br /> Improper riprap sizing or placement <br /> Excessive flow concentration <br /> Improper downdrain structures <br /> Possible approaches : <br /> Replace riprap per permit specifications <br /> Re-evaluate culvert and ditch relief spacing and provide additional <br /> culverts and/or rolling dips , with prope,- outlet protection . <br /> Install CMP or other protective downdrains for culverts which <br /> discharge high on fills or steep erosive downslopes such as those <br /> in the immediate vicinity of the portal benches . <br />
