Delaney; <br /> Enclosed please find a proposed settlement agreement for the above cited <br /> Notice of Violation. At Mid-Continent Resources ' request I convened an <br /> assessment conference in our offices to consider the above cited notice of <br /> violation on Thursday, December 19, 1991 . In attendance at the conference <br /> were Lew Thompson of Mid-Continent Resources, Inc. , and Steve Renner and Dan <br /> Mathews of the Division ' s staff, in addition to you and me. <br /> If you agree with the settlement agreement, please sign it and return it to <br /> our office as instructed on the form. The form must be signed within ten days <br /> of the above date. If you disagree, do not sign the form. The Division will <br /> fix the civil penalty as originally proposed and you may appeal that decision <br /> to the Board as outlined in the regulations , Rule 5.04.4.
