September 25, 2012 C -1996- 083 /13owie No. 2 Mine SLB <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Berms, straw bales, and a rock lined channel were in place at the Terror Creek Vent Shaft location. There <br />was no evidence of erosion or siltation coming from the vent shaft pad. The approved design, however, as shown <br />on Map 42 requires 25 linear feet of silt fence (2.5' ht). BRL should either install the required silt fence or revise <br />the permit application for straw bale filters. <br />Ponds B, C, D, and F were holding water, but none were being discharged. Ponds J and K were dry. <br />Hubbard Creek was flowing at the upper section, near the Hubbard Creek fan site. Flow was reduced to a trickle <br />at the lower end of the creek, near the Fire Mountain Canal. <br />PROCESSING WASTE /COAL MINE WASTE PILES — Rule 4.10 and 4.11 ❑ Drainage Control; Surface <br />Stabilization; Placement: <br />Refuse from the wash plant was being placed in Refuse Area 2. Grading and compaction operations <br />appeared to be contemporaneous with placement and in compliance with the approved plan. Lower benches were <br />being regraded and had been surveyed to detennine where additional grading was required to establish final grade <br />on the outslopes. <br />ROADS — Rule 4.0311 Construction 4.03.1(3)/4.03.2(3)❑ Drainage 4.03.1(4)/4.03.2(4)11 Surfacing and <br />Maintenance4.03.1(5) and (6)/4.03.2(5) and (6)❑ Reclamation 4.03.1(7)/4.03.2(7): <br />The operator has placed two 24" culverts in ditch D -1 to facilitate access to the cover material stockpile <br />at Refuse Area No.2. The operator indicated the culverts would remain for approximately 30 days. If the culverts <br />are not removed prior to October 31, 2012, the pen application package will need to be revised to include <br />design information for them. <br />The light use road to the Terror Creek Vent Shaft was in good condition. Gravel surfacing was adequate to reduce <br />sediment load from the road, and water bars were installed and maintained to carry water off of the roadway. <br />REVEGETATION — Rule 4.15 ❑ Vegetative Cover; Timing: <br />Perennial grasses were emerging on the reclaimed Hubbard Creek vent shaft site, but cheatgrass and <br />thistle continue to be a problem. Vegetative cover was heavier on the north end of the shaft site where mulch had <br />been applied. Cheatgrass was also heavy on the Dove Gulch site. The Division recommended that BRL consult <br />with Webb Callicutt, of Delta County, for information on pre- emergent herbicides to control the cheatgrass. <br />SUPPORT FACILITIES - Rule 4.04: <br />Facilities areas and associated yards were in good condition. Ditches and culverts were functional. Two <br />culverts were in need of maintenance - Culvert C -C20 (culvert that carries underdrainage below road fill, above <br />pond diversion gate) needed moss and other vegetation cleared at the outlet end, and C -C4 on the D Portal Bench <br />needed to be cleaned out in front of the inlet to prevent clogging of the culvert. <br />SIGNS AND MARKERS — Rule 4.02: <br />Number of Partial Inspection this Fiscal Year: 3 <br />Number of Complete Inspections this Fiscal Year: 1 <br />Page 3 of 7 <br />
