Metal Mining Stormwater Reapplication Q&A <br /> 1. Q: What is this all about? <br /> A: Your facility is certified under the Metal Mining stormwater general permit. Stormwater permits are issued for a five- <br /> year period,and expire after that time. This permit is about to expire. To maintain coverage,you must reapply. <br /> 2. Q: How do I reapply? <br /> A: We have tried to make the reapplication process as simple as possible;just sign the Reapplication and Record <br /> Verification form and send it back to us in the provided envelope. If anything has changed or is incorrect on the <br /> enclosed form,please cross out the old and write in the new. Please also provide the email addresses requested in items <br /> A and B,if available. (Make sure to save a copy of the form for your records.) <br /> 3. Q: Do I need to send any money? <br /> A: No-the billing process is handled separately. <br /> 4. Q: What if I don't need a permit anymore? <br /> A: Under either of the following conditions you may be allowed to inactivate your permit certification: <br /> o When a site has been reclaimed in accordance with the Colorado Division of Minerals and Geology(CDMG) <br /> requirements and the site has been released from State reclamation requirements,the operator of the facility may <br /> request termination. <br /> o In the case where the site has no CDMG permit,certification under this permit may be terminated the site is <br /> reclaimed in accordance with the specific criteria listed in Part I.F. of your current permit. <br /> To request inactivation, submit a competed Inactivation Notice. If you do not have a copy of the Notice, one may be <br /> obtained from the internet at www.cdphe.state.co.us/wq/penmtsunit or by calling our office at 303-692-3517 (please <br /> provide you;i:amc, mailing or email address, an, ::,,_licate you need tLe Metal ' Iinin`Storm eater Permit lnactivatior <br /> Form). <br /> 5. Q: What about my 2005 &2006 Annual Reports? <br /> A: The requirement to submit an Annual Report continues under the new permit. <br /> For active mining operations and mine remediation operations, your next Annual Report should cover all of 2006,and <br /> is due by February 15, 2007. Your 2005 Annual Report should have been sent to us by February 15,2006. For <br /> inactive mining operations, a reduced reporting frequency will once again be allowed for in the new pernut. Refer to <br /> the Annual Report requirements in the renewed permit when it is sent to you around September 30,2006. <br /> 6. Q: How do I ensure that my Stormwater Management Plan(SWMP)is in compliance? <br /> A: A guidance document on the contents of a SWMP required for compliance with the permit is available at: <br /> www.cdphe.state.co.us/wq/PennitsUnit/SWMetaIGuidance.pdf,or by calling our office at 303-692-3517 (please <br /> provide your name,mailing or email address, and indicate you need the SWMP Guidance for Metal Mining). The plan <br /> must be maintained by the permittee to accurately reflect the current conditions at the site,including the current <br /> potential pollutant sources present and Best Management Practices(BMPs)being implemented. Permittees must <br /> ensure their current SWMP is in compliance with these requirements prior to submitting the attached form and <br /> periodically throughout the permit term as necessary to ensure the plan remains up-to-date. <br /> 7. Q: What changes will be in the new permit? <br /> A: We are planning to make some relatively minor changes to the renewal permit. These changes will be summarized in <br /> the new permit Rationale,which you will receive with your renewed permit certification. We expect to send the <br /> renewal permit to public notice in late June 2006. We will not be sending out a separate public notice to Metal Mining <br /> pern ittees. If you would like to receive a copy of the permit or public notice,or submit comments on the permit,please <br /> call our office at 303-692-3517 in early July 2006. <br />
