A legible submittal is required. This <br /> map must be on paper 8 'h by 11 inches. <br /> Item 9- This is a facility sketch for inclusion in the permit. A legible submittal is required on paper 8 'h x 11. <br /> Item 10, 11,and 12- Self explanatory. <br /> Item 13- List in the table the outfalls expected,any treatment provided,the flows expected and the receiving stream. You <br /> may estimate the flow contribution if no data is available. For each treatment unit, indicate its design size, <br /> average flow rate and retention time and describe the ultimate disposal of any solid or liquid waste not <br /> discharged. Be sure to include the identity of the receiving water. Receiving waters are any waters of the state <br /> of Colorado. These include any and all surface waters that are contained in or flow in or through the state of <br /> Colorado(except for water withdrawn for use until use and treatment have been completed). This definition <br /> includes all water courses,even if they are usually dry. <br /> Item 14- Self explanatory. Specify the sample type and standard methods. <br /> Item 15- If land application,defined as any discharge being applied to the land, is practiced or proposed the Division <br /> needs appropriate information to understand the operation and make a judgment as to possible impact on ground <br /> or surface waters. If not identified elsewhere, identify the nearest surface waters or dry stream bed. <br /> Item 16, 17, 18, 19,20,and 21 -Self explanatory. <br /> Item 22- The application form shall be signed as follows: <br /> (a) In the case of corporations,by a principal executive officer of at least the level of vice-president or his or <br /> her duly authorized representative,if such representative is responsible for the overall operation of the <br /> facility from which the discharge described in the application originates. <br /> (b)In the case of a proprietorship,by a general partner. <br /> ® In the case of a sole proprietorship,by the proprietor. <br /> (d)In the case of municipal,state,or other public facility,by either a principal executive officer, ranking <br /> elected official,or duly authorized employee. <br /> Both the owner(s)and operator(s)shall sign the form. <br /> Dās"4ro3 <br />
