STATE OF COLORADO <br /> GENERAL PERMIT APPLICATION <br /> STORMWATER <br /> DISCHARGES ASSOCIATED WITH HEAVY AND LIGHT INDUSTRIAL ACTIVITY <br /> AND METAL MINING <br /> This application is for use by all industrial stormwater dischargers engaged in light or heavy industrial activity, <br /> metal mining, and coal mining (if not covered under the coal general permit, COG-850000), as indicated in <br /> Appendix A. This form may be reproduced. <br /> Application Due Dates: At least thirty days prior to the anticipated date of discharge, the owner (or operator <br /> if the owner does not operate the facility) of the facility shall submit an application as provided by the Water <br /> Quality Control Division(the "Division"). <br /> Permit Fee: Do not send any_nayment with this application. You will be billed once you are covered under a <br /> permit. <br /> Application Completeness: All items of the application must be completed accurately and in their entirety or <br /> the application will be deemed incomplete, and processing of the permit will not begin until all information is <br /> received. One original copy of the completed application(no faxes) shall be submitted, only to: <br /> Colorado Department of Public Health and Environment <br /> Water Quality.Control Division <br /> WQCD-Permits <br /> 4300 Cherry Creek Drive South <br /> Denver, Colorado 80246-1530 <br /> Attention: Permits Unit <br /> If you have questions on completing this application, you may contact the Division at (303) 692-3500. <br /> INSTRUCTIONS <br /> Item 1 - Provide the name and address of the permit applicant, including the company name, local contact, <br /> and mailing address. Indicate whether the applicant is the owner or operator of the facility. Include <br /> the applicant Federal Taxpayer Identification or Employer Identification number (this is a nine digit <br /> number in all cases). This number will be used as an identifier for billing purposes. <br /> Item 2 - For the approximate center point of the property, both types of descriptions (legal, in terms of <br /> Township, Range and 'A section, and longitude/latitude,to the nearest 15 seconds) must be included. <br /> This information is easily obtainable from a U.S. Geological Survey topographical map, available at <br /> area map stores, or call the USGS at (303) 202-4700. <br /> Item 3 - The Standard Industrial Classification (SIC) Code or codes for the facility must be listed. SIC <br /> Codes are assigned according to the primary activities performed by a company. They are often <br /> assigned for insurance purposes or when a business registers as a corporation. Industries can also <br /> determine their SIC Code by checking with their trade association, Chamber of Commerce, legal <br /> counsel, or local library for the SIC Manual.
