Mine Status (activities): <br />Energy Fuels Coal, Inc.' Southfield mine ceased all mining and coal processing activities in January, 2001. <br />The mine was placed on Permanent Cessation Status at that time, sealing of the mine and demolition of <br />facilities commenced during the same year. Permanent seeding, mulching and crimping of the mulch <br />were done during October, 2003 on the facilities and mine areas. The site is currently in Year 9 of the <br />Ten Year Reclamation Liability Period as per regulations of the Colorado Division of Reclamation, Mining <br />& Safety (DRMS). <br />Only limited activities occur on the site and the site is enclosed within locked gates. Activities consist of <br />routine inspections by the mine manager and a representative inspector of the DRMS. An occasional re- <br />grading of a section of drainage ditch or a section of light use road may be required for routine <br />maintenance. <br />Area Subject to ELG's — description: <br />All of the areas subject to effluent limitation guidelines (ELG's) are within the reclaimed areas of the site <br />and vegetation being in the ninth growth season as stated above, is well established minimizing <br />drainage /runoff. Surface water drainage and potential water discharge is monitored according to the <br />respective discharge permits and the approved water monitoring plans in the Energy Fuels Coal, Inc.' <br />mining permit, Permit No. C -81 -014 on file with the DRMS. <br />Site Map: <br />SWMP Administrator: <br />SWMP Plan (SWMP) and Spill Prevention Control Plan (SPCC) <br />Discharge Permit No. 850045 and 850006 <br />Energy Fuels Coal, Inc. — Southfield Mine <br />Fremont County, Colorado <br />A site map, Map SURFACE PRAIA/46E is enclosed with this submittal. <br />The Administrator of the Plan is the Southfield mine manager, George V Patterson and shall have the <br />responsibility implementing the plan and administering any corrective measures that may be necessary. <br />Identification of Potential Pollutant Sources and BMP's: <br />Given that former facilities and fuel storage have been removed from the site, potential pollutants are <br />limited to typically, four (4) each 5 gallon size cans of lubricating oil and a few tubes of lube grease. <br />These items are enclosed in steel cabinets with latched doors. As stated above, the site is enclosed <br />within locked gates. BMP's pertain to routine visit inspections by management and corrective action is <br />taken as required. Additionally, currently the DRMS is scheduled for inspections on a monthly basis. <br />
