PERMIT #: M- 2010 -034 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: September 13, 2012 <br />The site is permitted for 9.99 acres with 8.75 acres of disturbance and 1.24 acres of mine setbacks. The pre <br />and post mining land use for the site is rangeland. The site was not active during the inspection. The <br />approved mine plan states mining will progress from north to south and approximately 25 feet to material will <br />be removed from the hilltop creating a level floor with 3H:1V side slopes. The perimeter slopes will be mined <br />at 3H:1V and no backfilling will occur at the site. <br />The site is approximately 80 percent mined with only the southern hill and the eastern and western sides <br />remaining to be mined. Mr. Willits expects mining to be concluded by Fall 2012 or Spring 2013. A large <br />stockpile of screened rock was located at the north end of the pit floor. The pit floor consisted of red clay <br />material. No processing equipment was present during the inspection. An empty water tank was observed in <br />the middle of the pit floor and a berm for a fuel tank was noted, but no fuel tank was present during the <br />inspection. The approved mining offsets from the permit boundaries are 15 feet along the north and east <br />sides. The topsoil had been stripped in advance of mining activities. Topsoil stockpiles were observed along <br />the perimeter of the site in approximately the stockpile locations indicated on the approved Mining Plan map. <br />The stockpiles were vegetated and appeared stable. <br />As noted as a problem above, the required mine sign and boundary markers were not observed during the <br />inspection. The Operator stated the mine sign would be re- posted and the boundary markers would be re- <br />established based on GPS coordinates soon. The Operator will be required to submit photographic evidence <br />of the corrective actions completion within 30 days of the mailing of this inspection report. Due to the lack of <br />markers, the Division was not able to determine if the current mining disturbance was within the permit <br />boundaries. Once the boundaries markers are re- established, the Division may reinspect the site to verify the <br />mining disturbance is within the approved permit boundary. <br />Photographs taken during the inspection are attached below. <br />Page 2 of 4 <br />
