The permit for this mine was revoked and the <br />bond was forfeited. The reclamation was completed by the Division. Weather conditions were partly cloudy and <br />warin. <br />BACKFILL and GRADING — Rule 4.14 11 Contemporaneous Reclamation 4.14.1; Approximate Original Contour <br />4.14.2; Highwall Elimination 4.14.1(2)(f); Steep Slopes 4.14.2, 4.27; Handling of Acid and Toxic Materials <br />4.143; Stabilization of Rills and Gullies 4.14.6: <br />The backfilled portal area did not have signs of slumping or cracking. The soil above the lower fan portal <br />is collapsing. A sink hole has formed about 10 above the portal. Dense vegetation surrounding the sinkhole made <br />it difficult to inspect without getting too close. <br />FISH and WILDLIFE — Rule 4.18: <br />A dead long tailed weasel was on the upper bench. Its body was intact and a cause of death was not <br />obvious (photo 1). <br />HYDROLOGIC BALANCE - Rule 4.05 ❑ Drainage Control 4.05.1, 4.05.2, 4.053; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The cut bank on the upper bench area is collapsing into the interceptor ditch. The ditch is blocked forcing <br />water over the outer berm and down the face of the fill. This blockage has been reported in previous reports <br />(3/2012 and 10/2011). <br />REVEGETATION — Rule 4.15 ❑ Vegetative Cover; Timing: <br />The vegetation on the lower access road is good (photo 2).
