I, Jared Ebert of the Colorado Division of Reclamation, Mining and Safety conducted the inspection. <br />No one from the operation accompanied me during the inspection. The weather was warm and partly cloudy. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />A records check was conducted, the final page of this report includes the Division's checklist of the records <br />required to be maintained by the Operator at the site. Several of the records need to be updated with recent <br />documents. The following documents need to be updated; the DRMS Permit (from RN4), the Findings Document <br />for RN4, 2012 insurance certificate, latest DRMS inspection reports (May, June, July and August are missing), the <br />last NPDES records (1st Quarter, 2012 not included), and the 1st and 2nd quarter pond inspection reports for 2012. <br />HYDROLOGIC BALANCE - Rule 4.05; Drainage Control 4.05.1, 4.05.2, 4.053; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage —Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Pond B was dry and the embankment appeared to be stable. The site appeared to be very dry. <br />No spoil springs were observed in either Mine Area No. l or 3. <br />ROADS — Rule 4.03; Construction 4.03.1(3)/4.03.2(3); Drainage 4.03.1(4)/4.03.2(4); Surfacing and <br />Maintenance4.03.1(5) and (6)/4.03.2(5) and (6); Reclamation 4.03.1(7)/4.03.2(7): <br />All of the roads were driven and no problems were observed.
