The mine is a reclaimed underground mine <br />with no personnel remaining on site. The inspector had contacted George Patterson of Energy Fuels Coal, Inc. on <br />the way to the mine site on 8/24/2012. Mr. Patterson met the inspector on site and accompanied the inspection. <br />The inspection began on 8/21/2012 at the load out. A large storm at the end of July damaged a bridge on State <br />Highway 67. Highway 67 was closed for repairs on 8/21/2012, so the inspector was unable to access the mine <br />site. The highway was reopened on 8/24. the inspector conducted the mine site portion of the inspection of <br />8/24/2012. <br />The weather was cloudy on 8/24. Clouds were threatening to storm throughout the inspection on 8/24. <br />Intennittent showers passed through the mine site. There was evidence of sediment in the ditches, pond 5 and <br />along the access road that a large stone event had occurred previously at the mine site. There are no rain gauges at <br />the mine site to record precipitation events. <br />HYDROLOGIC BALANCE - Rule 4.05 Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />A large sediment fan had been deposited on the access road by the entry gate. Considerable runoff had <br />flowed down the ditch on the north side of the access road and deposited sediment just west of the access gate. <br />Some of the flow appeared to have originated from the slope north of the access road, but the larger flow path <br />appeared to have come from the road ditch. <br />Walking along the north side access road ditch found the apparent source of much of the sediment is a cut bank <br />along the road. This section of the cut bank is located west of the locked gate on County Road No. 92, but east of <br />the limits of the Southfield permit boundary. (within NW 1/4 SE14 Sec 19 T20S R69W).
