ADDENDUM, EXHIBIT D <br /> Mining Method Amendment <br /> The mining method employed for extracting targeted ores is shrinkage stoping, <br /> sublevel open stoping and blasthole stoping, depending upon the geometric <br /> configuration of the ore. <br /> Occasionally waste is mined in the development of target ores, and whew <br /> possible, the ore pass will be used for transferring the waste rock to the <br /> waste dump at the American Tunnel via rail haulage. <br /> Lake Emma Amendment <br /> In June of 1978, Standard Metals Corporation's Sunnyside Mine experienced <br /> the "Collapse of Lake Emma" which innundated the Sunnyside Mine with mud and <br /> water. <br /> The mine was reopened in 1978, 1979 and 1980 by cleaning out the mine and <br /> disposing of the mud at the Terry Tunnel and American Tunnel surface areas. <br /> The majority of all muds and waste that entered the mine have already been <br /> removed. <br /> In the summer season of 1981, Standard Metals conducted the following remedial <br /> actions: <br /> 1. Excavation of mud from the now dry lake bed and disposal in <br /> "mud disposal area". <br /> 2. Excavation of rock from a 72-foot deep trench and disposal <br /> in rock disposal areas. <br /> 3. Loading and road haulage of approximately 20,000 tons of ore <br /> from two old ore dumps in Sunnyside Basin. <br /> 4. Construction of a ring dike around the hole through which <br /> Lake Emma entered the mine and tying into the 72-foot trench <br /> in an attempt to capture 86% of the surface drainage in <br /> Sunnyside Basin before the water entered the mine. The <br /> construction material was borrowed from construction borrow <br /> area as shown on map of Lake Emma affected lands. <br /> Care and maintenance of the dam and site was conducted in 1982, 1983 and 1984. <br /> Annual care and maintenance included cleaning sediments from the trench and <br /> reestablishing the grade in the trench and ring dike to 1% grade and doing any <br /> repairs necessary to the dike. <br /> The area of disturbance including the major, moderate and minor disturbance <br /> categories is 29.99 acres. <br />
