OBSERVATIONS <br />PERMIT #: M- 1977 -252 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: August 2, 2012 <br />1) The Division conducted a monitoring inspection of the site on 8/2/12. Present during the inspection was Mr. <br />Jason McGraw of General Shale. The site is permitted to affect 34 acres of the total permit area of 69 acres. It <br />is located approximately 2 miles south of Parker Colorado. <br />2) The permit sign was properly posted at the access to the property. The entire permit area is fenced. The <br />affected area was walked and inspected. There was some water noted in the pit floors from the recent rain <br />event. All the drainage from the affected area is captured with in the pit. Pit high walls were stable. No <br />problem noted. <br />3) The current permit area covers a portion of two adjacent city roads. The operator wanted to know how to <br />remove these city roads from the permit area. He was informed to submit an acreage reduction request in <br />accordance with Rule 4.17 of The Construction Materials Rules and Regulations and the Division will release <br />the two areas from the permit. The release request must also include a new map showing the removal of the <br />roads from the permit area. <br />4) With the exception of the lack of a noxious weed control plan in place, the operation is in compliance with the <br />approved reclamation plan. The lack of a noxious weed control plan is cited as a possible problem with a <br />compliance date. <br />5) The current financial warranty in the amount of $ 91, 243.00 is deemed adequate at this time. <br />Page 2 of 4 <br />
