OBSERVATIONS <br />PERMIT #: M -1986 -147 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: July 17, 2012 <br />1) The Division conducted a monitoring inspection of the site on 7/17/12. The total permit area of the site <br />is 89.53 acres. Present during the inspection was Mr. Darren Gephardt the operator's representative. <br />2) The site was not active at the time of the inspection. Approximately 30 acres of the total 89.53 acres <br />appears to have been affected at the time of the inspection. The site was not active at the time of the <br />inspection. <br />3) The entire permit area was inspected and found to be in compliance with the approved reclamation <br />plan. Permit sign and boundary markers were properly posted. <br />4) The current financial warranty in the amount of $71,755.20 is deemed adequate to cover the cost of <br />the current affected area reclamation. <br />5) The operator was notified to submit a technical revision to address the lack of a noxious weed control <br />plan. Even though there was no large noxious weed infestation noted at the time of the inspection, a <br />noxious weed control plan is required. The lack of a noxious weed control plan is cited as a possible <br />problem on the first page of this inspection report with a resolution date. <br />Page 2 of 4 <br />
