OBSERVATIONS <br />PERMIT #: M- 1976 -056 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: July 17, 2012 <br />1) The Division conducted a monitoring inspection of the site while in the area conducting other inspections. <br />Present during the inspection was Mr. Darren Gephardt the operator's representative. The site was last <br />inspected on 10/27/09 as part of an acreage reduction release request. The release request was denied. <br />2) The total permit area for the site is 86.40 acres. Post mining land use is wild life habitat. The operator utilizes <br />dredging operation to remove the gravel from the pit. No dredging was taking place at the time of the <br />inspection. <br />3) The entire permit area was driven and inspected. The lake embankments were stable and vegetated. Some <br />noxious weed infestation was observed at the time of the inspection. The operator was notified to submit a <br />noxious weed control plan as Technical Revision to the permit. <br />4) Permit sign and boundary markers were properly displayed. The current financial warranty in the amount <br />of #131,200.00 is deemed adequate to cover the cost of reclamation at this time. <br />Page 3 of 5 <br />
