amet_-
encountering seventeen voids. An approximate total of 1,250 linear feet was drilled. As -built
survey of the drill holes was performed by Ames.
Grouting of missed (non -void) confirmatory drill holes was performed at Underground Workings
#4073 and #4081. A grout pump and laborers were used to place bentonite grout into the "miss"
drill holes.
C) Remediation
Remediation of Underground Working #4045 was performed placing the concrete plug in the
working. Seven feet of cemented rock fill was placed on top of the previously placed concrete. An
AMEC field representative observed cemented rock fill.
Remediation of Underground Workings #4052 and #4058 was performed excavating the crown
pillar from the working. A portion of the crown pillar was removed during the excavation. The
remaining crown pillar will be drilled and collapsed by blasting.
Remediation of Underground Working #4077 (The Wedge) was performed using a John Deere
excavator to remove timbers and unsuitable material from the working. Coarse shaft backfill
placement was then performed filling the open shaft. The working was backfilled to approximately
10 feet from the existing ground surface. The sides were then shaped with 05V:1 H slopes
preparing the shaft for concrete plug installation. An AMEC field representative observed the
backfill and inspected the shaped upper portion of the shaft. It was found to be acceptable for
concrete placement. Approximately twenty cubic yards of approved concrete from Transit Mix was
placed in the plug. The concrete placement was observed and tested by an AMEC field
representative. Slump, air entrainment, and temperature testing was performed with results
passing. One set of nine 4 "x 8" cylinders were cast for laboratory compressive strength testing.
The crown pillar at Underground Working # 4081 was collapsed by blasting. A CAT 330 excavator
was used to excavated the collapsed crown pillar from the working. An AMEC field representative
observed the remediation activities.
D) Structures
No work performed during the week.
III) Geosynthetics
A) Geomembrane Installation
No work performed during the week.
B) Anchor Trench
No work performed during the week.
C) Geomembrane Exposure
No work performed during the week.
D) Geomembrane Acceptance
No work performed during the week.
1V)Storm Water Management
A) Best Management Practices (BMP)
Best Management Practices are being observed throughout the Mill Site Earthworks project.
MEW Weekly Report WE 05.19.2012
