(Page 3) <br /> MINE ID # OR PROSPECTING ID# M-77-378 <br /> INSPECTION DATE 10/19/99 INSPECTOR'S INITIALS WHE <br /> Sunnyside Basin Not inspected. DMG last inspected the Sunnyside Basin area on 8/21/98 and <br /> reported the following: <br /> Lake Emma subsidence feature - operator efforts to stabilize subsidence feature, by backfilling with <br /> native fill material, continues. During the 10/19/99 inspection, the operator indicated that <br /> additional backfill may be required. <br /> Washington Pit - Reclamation completed in accordance with permit conditions, the Act and Rules. <br /> Sunnyside Pit - Reclamation completed in accordance with permit conditions, the Act and Rules. <br /> Terry Tunnel Mine identification sign and affected area perimeter markers were posted. Public <br /> access is controlled by locked gate, gate was open. Portal discharge, draining from the near <br /> surface fracture zone in front of the bulkhead, reported to the sediment ponds. Water from the <br /> ponds was being pumped back into the mine pool, behind the bulkhead, to ensure sufficient pond <br /> capacity for spring runoff. Two sediment ponds appeared stable, no evidence of inslope slumping, <br /> sliding, excessive erosion or seepage, no discharge noted. <br /> Mayflower Ponds 1. 2 & 3 Outslope of pond embankments appeared stable and well vegetated. <br /> Sediment containment ditches, located along the toe of the embankment outslopes, appeared <br /> stable and well maintained. Surface reclamation of the ponds appeared to have been completed <br /> in accordance with permit conditions, the Act and Rules. Upland diversion ditches A, B and C, <br /> constructed beyond the permit boundary under the Consent Decree, Upland Hydrologic Control <br /> Project, appeared well constructed and complete. Additional comments for the Upland Hydrologic <br /> Control Project are recorded under a separate report. <br /> Mayflower Pond 4 Pond 4 remains active to receive sludge from the American Tunnel water <br /> treatment plant. Composted cow manure had been distributed over portions of the embankment <br /> outslope to enhance revegetation efforts. Canada thistle infestation, located upslope of Pond 4, <br /> had been sprayed, live Canada thistle were not observed. Construction of the upland diversion <br /> ditches and groundwater intercept walls were complete. Operator indicated that additional ditch <br /> and intercept wall construction may occur, pending BLM activity for relocating tailings from BLM <br /> administered lands.
