• (Page 2) <br /> MINE ID # OR PROSPECTING ID # M-1977-378 <br /> INSPECTION DATE 8/17/01 INSPECTOR'S INITIALS WHE <br /> OBSERVATIONS <br /> This inspection occurred to verify preparations of the tunnel and bulkhead forms for bulkhead #2 <br /> in the American Tunnel, prior to concrete pour. Bulkhead #1 in the American Tunnel was approved <br /> under TR-14. Bulkheads #2 and #3 were approved under TR-25. <br /> The Sunnyside Mine is approved for 236.21 acres affected area. DMG holds $1,250,000 financial <br /> warranty. Operator has commenced final reclamation. Water treatment plant at the American <br /> Tunnel facility was active. <br /> Preparations for concrete pour for bulkhead 2 were near completion and appeared consistent with <br /> the plans submitted by the operator and approved under TR-25. Location of bulkhead 2 is <br /> approximately 2000' in from the portal opening. Tunnel walls, floor and roof at the bulkhead <br /> location had been cleaned of loose rock, sandblasted, washed and vacuumed. All portions of the <br /> cleaned tunnel appeared to be competent rock with sufficient surface irregularity for concrete <br /> contact. The operator indicated that a coffer dam had been constructed immediately upstream of <br /> the bulkhead location. The coffer dam was necessary to impound water to sufficient height to <br /> enter the drain pipe and route mine drainage through the bulkhead during construction. The <br /> operator also indicated that approximately 5 tons of hydrated lime had been placed between <br /> bulkheads 1 and 2 to buffer the pH of the water impounded behind bulkhead 2. <br /> Concrete forms were constructed from heavy timbers and treated lumber. The concrete forms <br /> fitted tightly with tunnel surface and were well braced with drill pipe and angle iron bolted to resin <br /> bolts. As approved in the plans, the bulkhead will be 10' long and approximately 13' wide. <br /> However, significant material had to be removed from the floor of the tunnel to ensure appropriate <br /> contact with competent rock. Therefore bulkhead 2 will be approximately 17' high rather than the <br /> 14' height anticipated in the approved plans. Rebar mat were installed 3.5" in from the concrete <br /> face at the upstream and downstream sides. Upstream rebar mat was constructed from #10 rebar <br /> with 6.5" centers for vertical and horizontal members. Downstream rebar mat was constructed <br /> from #10 rebar with 6" centers for vertical and horizontal members. Trim bars, #10 rebar at 45 <br /> degrees to the rebar mat, were installed in the corners to prevent corner cracking.
