OBSERVATIONS <br />PERMIT #: M- 2001 -005 <br />INSPECTOR'S INITIALS: TCI <br />INSPECTION DATE: June 4, 2012 <br />This was a regular monitoring inspection conducted in order to check compliance of the operation with the <br />requirements of the Act, Rules, and the conditions of the permit. The Operator, Continental Materials <br />Corporation was represented during the inspection by Andre Laroche. The Grisenti Farms Gravel Pit is located <br />approximately 2 miles south of Penrose, CO. The access road is about 1 mile northeast of the intersection of <br />State Highways 115 and 120. <br />Inspection: <br />A permit sign was prominently displayed at the site entrance (see Photo 1) and boundaries fairly well marked. <br />The site was inactive at the time of inspection. Mr. Laroche indicated mining is completed in Phase 1. The <br />Phase 1 Pond slopes have been graded, topsoiled and seeded. Current mining operations are now in Phase 2. <br />Topsoil stockpiles are labeled and stable (see Photo 2). Processed material stockpiles were observed. (see <br />Photos 3 and 4). The processing area appeared orderly. Screens, a jaw crusher, cone crusher and conveyors <br />were observed in the processing area. Fuel systems are protected with concrete containment walls and bollards <br />(see Photo 5). Mr. Laroche indicated chemicals are stored in a locked shed (see Photo 6) <br />Groundwater was exposed at the time of the inspection: Phase 1 Pond, Phase 2 Pond, and the Freshwater Pond <br />(see Photos 7 — 9). Mr. Laroche indicated the dewatering trenches are being maintained (see Photos 10 and <br />11). <br />A large infestation of Russian Olive and Tamarix was observed between Phase 1 and Phase 2, near Brush <br />Hollow Creek, and primarily east of the creek and north of the main access road (see Photos 12 — 14). This is <br />cited as Problem 1 on page 1 of this report. <br />File Review Notes: <br />• The total permitted area is 115.3 acres of which 99 acres may be affected area. <br />• The approval of TR- incorporated a weed control plan. <br />• The previous inspection was performed on 11/7/07. No problems were noted. <br />• DWR has approved SWSP ID 4625 through January 31, 2013. <br />• There is a $115,000 financial warranty (corporate surety) for the site. <br />• Annual fees and reports are up to date through 2012. The 2012 annual report indicates 58 acres are <br />disturbed (4.8 acres in the 2011 — 2012 report year). 13 acres have been backfilled, topsoil replaced and <br />seeded. <br />• The post -mine land use is wildlife habitat. <br />Current Stipulations: <br />None. <br />Recommendations: <br />The Division recommends the following tasks be completed by the Operator: <br />1. Remove the Tamarix and Russian Olive infestation from the site in accordance with the weed control <br />plan approved in 2005. <br />2. When vegetation is established in the Phase 1 area, contact the DRMS to request a surety reduction. <br />Page 2 of 5 <br />
