Although this annual proof of insurance was required to be <br />submitted to the Division when the Hayden Gulch Mine had an active permit with the Division, the <br />Division approved final bond release of the Hayden Gulch Mine on October 26, 2006. The permit was <br />terminated at the time of final bond release. Proof of insurance for this property is no longer required <br />by our agency. <br />Our records indicate that our office notified you of this circumstance on July 13, 2010 as well. I am <br />enclosing the certificate of insurance with this letter.
