Pre-pour Inspection 2 October 8, 1996 <br /> Gold Prince Interior Bulkhead Inspection date 9/23/96 <br /> SGC Consent Decree DMG file M-77-378 <br /> Rebar members were spaced on 12" centers. Rebar cages were spliced to resin bolts. <br /> Rebar members of cages were continuous, splice were observed only to resin bolt <br /> anchors. Horizontal and vertical rebar members were secured to one-another by wire <br /> wrap at 12" crossings. Rebar cages were spaced 4" from interior face of concrete forms, <br /> no rebar or splice wires were observed in contact with face of concrete forms. <br /> Materials and equipment, located at or near the portal pad, included the following: <br /> arc welder; compressor; concrete pump with lines & couplings; 2 mixing cones; <br /> 2 fresh water tanks, full, approximate volumes - 300 gal. & 500 gal.; <br /> 9 pallets of 60 lbs. bags, marked, "Burrell Concrete Products, 700 East Main <br /> Street, American Fork, Utah 84003". Mr. Morris stated that Burrell Concrete <br /> product was prepared specifically for sulfate water conditions encountered at Gold <br /> Prince bulkhead; and <br /> 2 pallets of 94 lbs. bags, marked, "Low Alkali Type 5 Cement, Rio Grande Portland <br /> Cement Corp." <br /> Mr. Morris indicated that approximately 13 cy of concrete were required for the Gold <br /> Prince bulkhead. Approximately 1 week following completion of pour, grout holes will be <br /> drilled between the rebar members to the bulkhead/parent rock contact.
