See attached location and site map. <br /> Latitude 37 degrees 54 minutes Longitude 107 degrees 43 minutes <br /> Identification of the Waters of the United States Potentially Impacted <br /> Headwaters of North Mineral Creek. Mineral Creek flows into the Animas River below Silverton Colorado. <br /> See attached map. <br /> 2)StteMao <br /> Attached <br /> 3)Stormwater Management Controls <br /> Prior to draining of the pond, the pH will be raised to minimize the metal content of the pond water. <br /> Downstream sediment catchments will be installed to minimize downstream effects during remediation. <br /> 4)Inspection and Record Keeping Procedtms <br /> The Manager or a member of the Technical Services Department will inspoct this project prior to and during <br /> remediation efforts on a regular basis.Quarterly reports and photographs will be sent to both the Colorado Water Quality <br /> Control Division and the Colorado Division of Minerals and Geology. Price to remediation efforts the area will be <br /> photographed and the photographs will be submitted with the first quarterly report.Reports for this project will include <br /> finalized engineering drawings, with estimates of operation, maintenance and replacement requirements for a passive <br /> bio mass treatment system. <br /> Once all reclamation activities included in the plan are complete,a final report will be submitted <br /> Monitorin¢ <br /> Monitoring points for this project will consist of upstream,downstream as well as flows from adits,seeps and <br /> dumps. Monitoring will start in 1996 and continue until two years after the project is completed. Monitoring for run <br /> on, runoff and waste isolation will occur four times yearly, and particularly once in high flow and twice in low flow. <br /> All samples taken will be analyzed for dissolved metals Zn,Fe,Al, Mn,Cd,and Cu: total sulfate,hardness and a field <br /> pl-I will be taken as well as field measurements for flow.All monitoring results%gill be submitted by the 28th day of the <br /> month following receipt of analytical results as well as included in the quarterly reports. <br /> 5)Mine Remediation Plan <br /> Legal Right to Enter and Conduct Activities <br /> SGC has verbally discussed Qus concept in general terms with the property owner. No work will commence <br /> until an acceptable access agreement is reached. <br />
