1
(Page 3)
•MINE ID # OR PROSPECTING ID # M-77-378
INSPECTION DATE 6/28/95 INSPECTOR'S INITIALS ACS
failure by thrust. It was observed during this inspection that considerable intact rock
irregularity results in the intrusion of intact rock into the bulkhead area perimeter.
Scaling and washing of the bulkhead location was very well done. No loose rocks remain, and
the location was free of dirt or residue. This degree of preparation will maximize the bond
between the concrete bulkhead and the surrounding rock. No adverse geological structure was
observed at the bulkhead location. The few fractures that are present are healed by a light
colored mineral in-filling.
The methods involved in batching the concrete and transporting it to the bulkhead location
were discussed, and the equipment inspected. As with all the bulkheads, fresh water from
Boulder Creek (the same creek that provides Silverton drinking water) will be used to mix the
concrete. The final test of the ability of the bulkhead to function as designed will come
out of the 7-day and 28-day concrete strength test results, and from the success of the
required low-pressure grouting of the bulkhead. The Division is satisfied that the operator
has considered every reasonable contingency, and has taken every necessary precaution to
ensure a successful pour. The operator has met or exceeded the Division's requirements for
site preparation.
I & E Contact Address
❑ CE
NAME Larry Perino ❑ BL
OPERATOR Sunnyside Gold Corporation ❑ FS
STREET P.O.
