C' <br /> DATE: May 16, 2000 <br /> TO: Interested Contractors <br /> FROM: Steven Renner, Division of Minerals and Geology <br /> RE: Documented Quote, Coal Basin Weed Control Project(PKA-O-XXXX) <br /> Enclosed is a brief description, site map and bid schedule for the Coal Basin Weed Control <br /> Project. NOTE that applicator must possess a valid certification. A copy of the certification must <br /> be presented to the PROJECT MANAGER prior to the project beginning. <br /> Site Showing <br /> There will not be a pre-for this project. However, the area to be treated under the scope of this <br /> Project is very discontinuous. CONTRACTORS are encouraged to visit the site prior to bidding. <br /> Bid Submission and Evaluations <br /> Bids shall be submitted by 2:30 p.m. on June 6, 2000 by fax, mail or in person to Yvonne <br /> Brannon. Telephoned bids will not be accepted. The attached bid schedule must be submitted <br /> by all bidders. Award will be made to the lowest grand total bid. <br /> Bid Award <br /> The apparent low bidder must submit a certificate of insurance with the following limits: <br /> 1. Standard Worker's Compensation and Employer Liability, as required by State Statute <br /> including occupational disease, covering all employees at the work site. <br /> 2. General Liability (Public Liability& Property Damage) (Minimum); <br /> a. Combined single limit of$600,000 written on an occurrence basis; <br /> b. Any aggregate will not be less than $1 million; <br /> C. The contractor must purchase additional insurance if claims reduce the annual <br /> aggregate below$600,000; <br /> d. The State of Colorado must be named as additional insured on each <br /> comprehensive general liability policy; <br /> e. Certificates of insurance must be provided to the State within 10 working days <br /> after receipt of Award; and, <br /> f. Insurance shall include provision providing notification to the State 10 days prior <br /> to cancellation of the coverage. <br /> 3. Automobile Liability (Minimum). The contractor must carry a minimum of$600,000 <br /> combined single limit. <br /> 1 <br />
