DATE: April 30, 2002 <br />TO: <br />FROM: <br />Matt Johnson, Roaring ork -ge -t;• Management <br />Steve Renner, Division of in-rals and Geology <br />RE: Work Description; Coal Basin 2002 Weed Control Project (PKA- 2 -XXX) <br />Enclosed is a description and Bid Schedule for the Coal Basin Weed Control Project. <br />The Bid Schedule and list of equipment offered must be submitted by 2:30 p.m. on May 15, <br />2002 by FAX, mail or in person to: <br />Steve Renner, Division of Minerals and Geology <br />101 South 3rd Street, Room 301 <br />Grand Junction, Colorado 81501 <br />FAX: 970 - 241 -1516 <br />Award will be made if the amount bid is within the Project budget estimate, and if the equipment <br />and personnel offered are deemed sufficient by the Division to accomplish the Proiect within the <br />specified time of completion. <br />List of Equipment Offered. The List of Equipment Offered form must be returned with the Bid <br />Schedule. All equipment to be used on the project must be listed. The number of laborers, <br />including equipment operators, who will be made available to perform the specified work for the <br />purpose of completing the Project within the specified completion time, must be included on the <br />List. <br />The CONTRACTOR must own, have a current lease for the equipment intended to be used to <br />perform the work, or must furnish a statement of arrangements to rent, or otherwise procure, <br />adequate equipment to perform the work. Inclusion of a piece of equipment and inclusion of a <br />work crew size on the List of Equipment Offered represents a commitment to devote that piece <br />of equipment and that number of laborers to the project work for the entire duration of the <br />project. If several contractors propose to join together to perform the work under this purchase <br />order contract, equipment owned or controlled by each should be listed and the owner <br />identified. A purchase order contract will not be awarded to a Contractor who in the opinion of <br />the OWNER (a) does not have adequate equipment, (b) does not have an adequately sized <br />work crew, or (c) does not have firm and satisfactory arrangements to obtain adequate crew or <br />equipment to perform the work described in the Project Description. <br />Contractor MUST submit a Certificate of Insurance with the following limits by June 5, 2002: <br />1. Standard Worker's Compensation and Employer Liability, as required by State Statute <br />including occupational disease, covering all employees at the work site. <br />1 <br />
