The Division has completed its review of the above referenced report received on April 2, 2012. The
Division finds the report to be in general compliance with Rule 2.04.13 and the permit. However, there
are several items that will need to be addressed as discussed below.
Annual Report Form:
1.) The permit number listed on the report form indicates the permit number is C -94 -081. The correct
permit number is C- 1992 -081. Please send a revised page with the correct permit number.
2.) On the ARR form submitted; under the " For Government Performance and Results Act it
indicates the acreage in long -term mining or reclamation facilities to be 83 acres. This area included the
total disturbed area (97.1 acres) minus the acres backfilled, graded, topsoiled and seeded (14.1 acres).
As of this time, none of these areas have been released from bond liability, so the area reported for this
section should be the total 97.1 acres.
2.04.13(1):
(a)- The narrative of the ARR included the name of the permittee, the address and the permit number.
(b)- No additional acres were disturbed during the calendar year 2011.
(c)- 12.7 acres were backfilled and graded during 2011. These areas included 11.9 acres in the former
loadout facilities area and .8 acres associated with Topsoil Pile TS -1.
(d)- 12.7 acres were topsoiled during 2011. These areas included 11.9 acres in the former loadout
facilities area and .8 acres associated with Topsoil Pile TS -1. Three topsoil samples and one subsoil
sample were taken an analyzed.
