SUBJECT: Limited Commercial/Public Applicator Registration
Your registration expires on December 31 of this year. If you intend to use restricted use pesticides or
wish to renew Hour registration voluntarily, please note the following procedures
1 Review each page of the attached application for accuracy and make any necessary
corrections;
NOTE: Please read, review. and answer all questions caiefulk If any question is answered
incorrectly or the required information is not SLIhInitted, the application will be retained
2 If you add a new qualified supervisor or certified operator or delete the same, please be
advised that this office requires a"Notification of Qualified Supervisor or Certified
Operator Form" (DPI-PA-58)be completed before they can be listed or deleted from your
registration. This form needs to accompany the application ONLY if changes are made to

4. Attach the registration fee of$50.00
Make checks payable to the Colorado Department of Agriculture,
5. Enclose a NENV and CURRENT Certificate of Good Standing.
Puhiic applicators and sole-proprietorships are NOT required to provide a certificate of
good standing. A certificate of good standing is required by the Rules issued
under Title 34, Article 10 for all corporations and other business entities for
which the Secretary of State provides such documentation. The company's
standing MUST be reaffirmed each year. Certificates of good standing that were
issued over 60 days prior to an applicant's application will not be accepted.
6. A certificate of Good Standing may be requested from the Secretary of State's Office
through their webb site at A,ww.sos.state.co. Return the completed registration
form, the registration fee, and new certificate of good standing (if applicable) to
the above address. Your new registration and endorsement will be mailed to you
