PERMIT #: M -1999 -021 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: May 31, 2012 <br />The Division observed several product stockpiles located at the east side of the pit. The product stockpiles <br />include 1/2" gravel, sand and road base. The scale and scale house have been removed from the site. The <br />Operator estimates there is approximately 50,000 to 60,000 tons of pit -run remaining to be mined from the <br />pit. As noted in the previous inspection report, the pit slopes on the north, south and west sides of the pit <br />have been graded to 3H:1V; the pit slopes are stable and did not show any signs of erosion. <br />Revegetation: <br />To date the Operator has not reseeded any portion of the affected area. The Division observed Russian olive <br />along the south side of the pit. In addition, field bindweed was observed throughout the site. Both of these <br />weed species are on the Colorado Noxious Weed List and require management by the Operator. The presence <br />of noxious weeds has been cited as a problem and will require corrective action by the Operator; please see <br />the first page of this report for additional information. <br />Signs and Markers: <br />The Operator did not have a mine identification sign posted at the site entrance as required by Rule 3.1.12. <br />This has been cited as a problem and will require corrective action by the Operator. Please see the first page of <br />this report for additional information. <br />Topsoil: <br />During the inspection the Division observed a topsoil stockpile which appeared to have been recently placed <br />on the site. According to the Operator, the topsoil was imported from an offsite location. Approximately 6,000 <br />cubic yards of the topsoil was imported at the end of 2011. Pursuant to Rule 3.1.5(9), if an Operator intends to <br />backfill inert structural fill generated outside of the approved permit area, it is the Operator's responsibility to <br />provide the Division notice of any proposed backfill activity not identified in the approved Reclamation Plan. <br />The notice to the Division shall include but is not limited to: <br />a) a narrative that describes the approximate location of the proposed activity. <br />b) the approximate volume of inert material to be backfilled <br />c) a signed affidavit certifying that the material is clean and inert, as defined by Rule 1.1(20) <br />d) the approximate dates the proposed activity will commence and end, however, such dates will not be an <br />enforceable condition. <br />The importation of topsoil from offsite has been cited as a problem and will require corrective action by the <br />Operator. Please see the first page of this report for additional information. <br />In addition to the imported topsoil, the Operator has stockpiled topsoil along the west and north sides of the <br />pit. The topsoil stockpiles are vegetated and did not show any signs of erosion. <br />This concluded the inspection. <br />Page 4 of 6 <br />
