June 4, 2012 X-2009-231-00/Sage Creek Mine KAG

Site 43 was found late in the afternoon on June 5 in an area of CRP pasture in Section 22 T5N R87W.
While difficult to locate immediately, an aluminum cap was found upon closer inspection. The site was
oo
well reclaimed and stable. It is located at approximately 40
Sites 38, 40, 42, and 44 were viewed using remote imagery. Nothing unusual was observed.
Eighteen geotech holes were drilled in an area around the currently developed Sage Creek mine portal.
Six of these holes, CCUGT002W, CCUGT003W, CCUGT004W, CCUGT005W, CCUGT011W, and
CCUGT013W were developed into water monitoring wells and are apparently covered under the Sage
Creek mining permit. The remainder of the holes were sealed according to reports filed with the Division.
Please provide information regarding specific well names or other identification to positively determine
whether these wells are included in the reclamation cost estimate for permit # C-2009-087 so that the
Division can confirm they are presently double bonded.
Assuming these geotech holes are bonded under the Sage Creel permit, field inspection indicates the
bond can be fully released for this project.

Number of Partial Inspection this Fiscal Year: 0
Number of Complete Inspections this Fiscal Year: 0

35
Page of
