The required $257 fee has been received and <br />processed under receipt no. 3649013. The most recent approval for a substitute water supply plan <br />( "SWSP ") is dated June 30, 2010 for a plan which expired on January 31, 2011. This SWSP <br />request was submitted according to C.R.S. § 37 -90- 137(11). <br />A well permit has been issued for this gravel pit under permit no. 61403 -F. On May 18, <br />2011 the owners on record (Carl Herskind and Gerald Hermans) were notified via certified mail that <br />they had 20 days to provide the required documentation showing evidence of construction of the <br />pit, along with a $30 filing fee, to this office. On June 6, 2011, the well owners submitted drawings <br />of the pit, a Pump Installation and Test Report on Form No. GWS -32, and the statutory filing fee of <br />$30; therefore, it has been determined that this permit is valid and shall remain in force and effect. <br />PLAN OPERATION <br />Transit Mix has a lease agreement with the Grisentis to mine sand and gravel from about <br />100 acres of land along the Arkansas River east of Florence. The completed Phase 1 mining <br />operation covered 35 acres. The Phase 1 pit was filled with water by an administrative exchange <br />during December 2009 resulting in a 15.62 acre pond whose surface is in equilibrium with the <br />water table. The Phase 1 pond will serve as the settling pond for the Phase 2 operation that is <br />currently underway. The attached Figure 1 shows the project phases.
