Pineda <br />Director <br />The Colorado Division of Reclamation, Mining and Safety (Division) has received the application <br />for Minor Revision 151 (MR -151) for the Deserado Mine. The minor revision was received on 29 <br />May 2012 and deemed complete on 8 June 2012. The Division has reviewed the submittal and has <br />the following adequacy questions. <br />Rule 2.05.3 — Operations Plan <br />1. The text on revised Permit Page III -35, in Section III.F, states that, at the rock dust hole <br />locations, separate 10 -inch drill holes are constructed to conduct breathable air to the <br />underground rescue chambers. However, the cover letter states that the breathable air hole <br />at the RDH -4 pad will be an eight inch hole. Please clarify the diameter of the breathable <br />air hole at the RDH -4 pad and correct either revised Permit Page III -35 or the cover <br />letter. <br />2. The text on revised Permit Page IV -4, in Section IV.13.3, includes a reference to the new <br />RDH -4 by location but not by name. The other rock dust holes include both a reference to <br />their location as well as their name. Please include a reference to RDH -4 by name as well <br />as by location in the text on revised Permit Page IV -4. <br />The text on revised Permit Page IV -56, in Section IV.P, provides a general location of the <br />three rock dust holes at the Deserado Mine. This text includes RDH -2, RDH -3 and RDH -4. <br />In the currently approved text, on Page IV -56, there is a reference to RDH -1, which was <br />located at the mine site. Please provide the Division with a brief discussion regarding <br />the status of rock dust hole RDH -1.
