The contract will be signed on July 25, 1995 at 4:00 p.m. To <br /> complete the process of bid award, the following information must be received by me in the offices of the <br /> Division of Minerals and Geology on or'before the date and time set for signing: <br /> 1) Five executed copies of the enclosed agreement, dated on the signing date. <br /> 2) A certificate of insurance and Additional Insured Endorsement, in the amounts given in Article <br /> 3, INSURANCE, of the General Conditions. The certificate of insurance must show the State of <br /> Colorado as an additional insured. <br /> 3) A performance bond and a labor and materials payment bond in the amounts of 100 percent and <br /> 100 percent respectively of the bid amount ($188,200). The bonds must be submitted using <br /> the State forms and must be dated the same day as the Agreement (the contract signing date). <br /> Copies are enclosed. <br /> 4) A project schedule, as specified in Article 5 PROJECT SCHEDULE of the General Conditions. <br /> 5) A completed AML Contractor Ownership and Control form Information Package. <br /> 6) A completed Minority/Women Business Participation form. <br /> 7) All copies of the enclosed three-way Agreement for Reclamation Activities, signed. <br /> 8) A blank original W-9 form is attached. <br /> When the above documents have been submitted and found to be in order, and the contract agreement fully- <br /> executed, the Notice to Proceed will be given.
