Re: Notice of Award, Coal Basin Mine 3 Project
Bid Number: PKA-5-359
Dear Sir or Madam:
This letter is to inform you that your company is the apparent low bidder on Project
PKA-5-359, the Coal Basin Mine 3 Project. The contract will be signed on July 25, 1995 at 4:00 p.m. To
complete the process of bid award, the following information must be received by me in the offices of the
Division of Minerals and Geology on or'before the date and time set for signing:
1) Five executed copies of the enclosed agreement, dated on the signing date.
2) A certificate of insurance and Additional Insured Endorsement, in the amounts given in Article
3, INSURANCE, of the General Conditions. The certificate of insurance must show the State of
Colorado as an additional insured.
3) A performance bond and a labor and materials payment bond in the amounts of 100 percent and
100 percent respectively of the bid amount ($188,200). The bonds must be submitted using
the State forms and must be dated the same day as the Agreement (the contract signing date).
Copies are enclosed.
4) A project schedule, as specified in Article 5 PROJECT SCHEDULE of the General Conditions.
5) A completed AML Contractor Ownership and Control form Information Package.
6) A completed Minority/Women Business Participation form.
7) All copies of the enclosed three-way Agreement for Reclamation Activities, signed.
8) A blank original W-9 form is attached.
When the above documents have been submitted and found to be in order, and the contract agreement fully-
executed, the Notice to Proceed will be given. If you have any questions, please give me a call me at 866-3567.
