Data Reporting (Continued) <br /> b) Quarterly <br /> Monitoring results obtained during the previous 3 months shall be <br /> summarized for each calendar month and reported on division <br /> approved discharge monitoring report forms, received at this office <br /> no later than the 28th day of the month following the completed <br /> quarter. For example, for the lst quarter (January, February, and <br /> March) the 3 monthly reports must be received at this office no <br /> later than April 28.
